   • Eastern School District Student Information Sheet (for returning students, please check information for accuracy; for new students, please fill in all information. NOTE: please include your parent's email on the bottom of the information sheet

   • Low Risk Activities consent form - take home to get signed; return tomorrow

   • Internet/Computer Usage policy - if you signed this in grade 7, you do not need to fill in again; only for new students

8. Scent free policy - body wash/spray, food allergies

9. Student fees - pay ASAP ($20 for 1 child, $25 for 2 children, $30 for 3 children)

10. Locker assignment/expectations for cleanliness

11. Student Phone - for emergency use only; must have yellow permission slip

12. Revised Bell Schedule

13. Attendance - parents should call the school and a note must be given to the homeroom teacher upon return
